Publications/Manuals (Drivers)

If you hold a Commercial Driver's License  you may apply in person at a Vermont Department of Motor Vehicles Customer Service Office, or through the mail for a new photo license.  You must complete an application for a duplicate or corrected CDL (PDF document 121.21KB) and submit it to this Department together with the appropriate fee and any additional documentation that may be required.

If your Vermont License, Learner's Permit, or Non-Driver ID has been lost, stolen, destroyed, or needs a correction to the information on it you may apply for a replacement license/permit/ID.  You must complete an application for a duplicate or corrected license/permit/ID (PDF document 136.13KB) and submit it to this Department together with the appropriate fee and any additional documentation that may be required.  This may be done either through the mail or in person at any of our office locations.

If you obtained a photo license/permit/ID on or after January 5, 2004 you can request a replacement photo license/permit/ID through the mail.

Note: When applying in person for a replacement license/permit/ID you must be prepared to provide required identification documents.  Additional documentation may be required (at the Department's discretion) if the documentation you submit is questionable, or if there is a reason to believe you are not who you claim to be.
