Notify the DMV of a Change of Address and/or a change of Name

You are required (by law) to notify the Department of Motor Vehicles within 30 days of any change in your mailing address, legal name or legal residence.

None of the following options apply to the holder of a Commercial Driver's License, Enhanced Driver’s License or Enhanced Non-Driver ID. If you hold a Commercial Driver's License you must complete form TA-VL-35 (PDF document, 121.21KB) and apply in person at a Vermont Department of Motor Vehicles Customer Service Office for a new photo license. If you hold an Enhanced Driver’s License or Enhanced Non-Driver ID you must complete form TA-VL-11 (PDF document,137.8KB) and apply in person at an EDL issuing office with the required documentation.

Option #1: Send (address change only) electronically through DMV EXPRESS

DMV Online Services:

Log on to DMV Online Services using the ID number that has been assigned to you by the Vermont DMV.

Your ID number is the same as your Vermont Driver License number. Your ID number can be found on your driver license or registration renewal certificate.

When you log on to DMV Online Services by entering your ID number, you will get a complete listing of all vehicles registered to you. On the same page you will find a link to web pages that will guide you through the process of changing your address on-line.

NOTE: If you log on to DMV Express using your vehicle's plate number you will not be offered the option of changing your address.

Option #2:  Process (address change only) electronically at a Vermont DMV KIOSK

The process for completing an address change on one of the Vermont DMV kiosks is similar to the process described above for DMV Online Services.

Vermont DMV kiosks are currently available at the following locations:  Bennington, S. Burlington, Montpelier, Newport, Rutland, and Springfield.

Option #3: Print (name and/or address change) form & submit

The name/address change form may only be accessed using Adobe Acrobat Reader. This form may not display and/or print correctly if you are not using the most recent version of Acrobat Reader. If you are having problems viewing/printing this form  please check the version of Acrobat Reader you are using.

If your name has changed the following additional documentation is required:

For an individual:  An individual's name change will require an original or certified copy of a marriage license/certificate, civil union certificate, or court order clearly stating your new name.

For a company:  A change of company name (on a registration) will require a change of name certification from the Office of Secretary of State.
