CDL Medical Certification

Federal Motor Carriers Safety (FMCSA) Regulations require all CDL holders to self-certify as to what type driving he/she performs or expects to perform.

Medical Evaluation Report

To be completed by a licensed physician when applying for a permanent disabled parking placard, disabled plates, and/or when a medical form is requested by the department to be submitted to the Department of Motor Vehicles.

CDL Medical Requirements

Physical Examinations

If you are subject to the Federal Motor Carrier Safety Regulations, you must have a physical every two years.

To have a hazardous materials endorsement, you must meet the Federal Motor Carrier Safety regulations except for age requirements for intrastate travel.

Physical must be performed by a nationally certified examiner.
